Postal Life Insurance Death Claim: Documents, Form & Nominee Checklist

Postal Life Insurance (PLI) या Rural Postal Life Insurance (RPLI) policyholder की मृत्यु होने पर nominee या अन्य eligible claimant को claim process के लिए official death claim form और supporting documents जमा करने होते हैं। Claimant की स्थिति और policy record के अनुसार अतिरिक्त documents माँगे जा सकते हैं।

1. Official death claim form

PLI Customer Portal – Download Forms से Death Claim Form प्राप्त करें। Form में claimant और policy details भरने से पहले instructions ध्यान से पढ़ें।

2. Documents checklist

• Original policy bond या official instructions के अनुसार Letter of Indemnity.
• Death certificate की self-attested copy.
• Claimant(s) का self-attested identity proof.
• Claimant(s) का address proof, जैसा current form में माँगा गया हो।
• भरा हुआ Death Claim Form और policy-specific additional documents.

3. Claim जमा करने की तैयारी

1. Policy number, policyholder name और policy status verify करें।
2. Nomination details policy record से मिलाएँ।
3. Death Claim Form में सभी जानकारी सही भरें।
4. Original bond/indemnity और claimant documents current instructions के अनुसार जमा करें।
5. Receiving acknowledgement और claim reference सुरक्षित रखें।
6. किसी document की कमी हो तो servicing PLI office से लिखित checklist लें।
